Want enthusiastic people who desire to help adults with chronic conditions learn to take better care of their health in a 6 week (2.5hrs per week) workshop developed by Stanford School of Medicine, in an interactive small group setting. Sponsored by SF Healthier Living Coalition of about 10 organizations incl. SF Dept of Aging/Adult Services, 30th St. Senior Center, St. Francis Hospital, Mercy Housing, NCPHS, SCAN, Self Help for Elderly, Curry Senior Center, SFHeathPlan.....
After training, facilitate workshops to help participants learn how to self mange their condition, problem solve to make positive changes, deal with difficult emotions, make weekly goals, be more fit and eat healthier, improve communication with doctors and family.....
